Discounts, promotions, sales, and membership services cannot be combined with any other offers. Discounts do not apply to add-on services. All offers are subject to their own terms and conditions.
Discount for skincare products can be used for member, friend, and family.
All membership plans require a 12-month commitment. To cancel, written notice must be provided at least 30 days before the end of the 12-month term.
If services remain unused for two or more consecutive months, they may be eligible for an upgrade to a higher-tier service, subject to availability. Please speak with staff for more information.

Does using the 4EvaGlow website create a patient-provider relationship?
No. Using our website or contacting us online does not create a medical or patient relationship. Treatment eligibility and results vary by individual and require an in-person consultation with a licensed provider.
Who can use 4EvaGlow’s website and services?
You must be at least 18 years old to access or use our website and services. By doing so, you confirm that you have the legal authority to agree to these Terms.
How does account registration work?
Certain features may require an account. You must provide accurate and current information, keep your login details confidential, and not share or transfer your account. 4EvaGlow may suspend or terminate access if misuse or false information is detected.
How do appointments and payments work?
All appointments are subject to availability and provider evaluation. Payment is due at the time of service unless otherwise stated. Accepted payment methods include cash, credit/debit cards, and approved financing partners.
What is included in each membership plan?
Glow Facial ($79/month): One monthly facial or small laser service, plus discounts on skincare, injectables, and more.
Glow Laser ($159/month): One monthly laser service for hair removal, skin treatments, or Glow Facial, plus deeper discounts and perks.
Glow Platinum ($399/month): Advanced monthly skin rejuvenation treatments like RF microneedling, laser resurfacing, or IPL, plus two additional services from Glow Facial or Glow Laser.
Can I roll over unused services?
Membership services must be used within the month; they do not roll over. This helps you stay consistent with treatments for the best results.
Can I share my membership with someone else?
Membership benefits are non-transferable. However, you can purchase skincare products with your member discount for friends or family.
Do memberships include Botox, Fillers, or Injectables?
Yes! All members receive 10–15% off Botox, Fillers, and injectables depending on their plan.
What happens if I want to upgrade my membership?
You can upgrade to a higher-level plan at any time. Your new benefits will begin the following billing cycle.
Is there a contract or cancellation policy?
Memberships are month-to-month. You may cancel anytime with a 30-day notice.
Do I get anything special for my birthday?
Yes! Each membership comes with a birthday credit:

Is there a minimum commitment period for memberships?
Yes, a 6-month initial commitment is required. After that, memberships continue month-to-month with 7 days’ notice to cancel before billing.
What happens if I don't use my monthly service?
Your unused service rolls over for 1 additional month if your membership stays active. Services expire if not used within the rollover period.
Can I combine membership discounts with other promotions or packages?
Membership discounts cannot be combined with other promotions, sales, or packages. They are exclusive to members.
Are there any additional perks for members?

How do I cancel my membership?
Submit a written cancellation request at least 7 days before your next billing cycle, after fulfilling the initial 6-month term. Membership fees are non-refundable once processed.
Can I upgrade or downgrade my membership tier?
Downgrades are available after the 6-month term or sooner with an adjustment fee or contract extension. Contact us to discuss options.
Are treatments transferable to friends or family?
Membership services and benefits are non-transferable. You can purchase gift cards separately for friends and family.
